ABPA is a complex pulmonary disorder caused by immune reactions mounted against Aspergillus spp. ABPA is diagnosed in those with predisposing conditions or compatible clinical presentation, with the mandatory presence of fungal sensitization and serum total IgE ≥500 IU/mL and any two of the following: fungal-specific IgG, peripheral blood eosinophilia, or suggestive imaging. Treatment response is assessed at 8-12 weeks using a multidimensional criterion.
